Library Resources Form for New Program/Course Proposals

Faculty Responsibility: You must provide complete item information and specific titles for all requested materials. Library staff cannot compile lists of needed resources for your program/courses - only you know which materials are required for your classes.

Library resources not needed for program/course

Inadequate library support will not delay approval. If support is adequate, additional materials may still be recommended. The Library is not responsible for supporting the course if "Library resources not needed for program/course" is chosen.

Books/Electronic Resources/Other

Please list key titles, whether or not the library already owns; attach program/course reading list, if any; library materials to be placed on reserve; wish list. If reading list not yet compiled, send ASAP. Attach additional sheet(s) if needed.

Key Journal Titles Needed/Recommended

Please submit tentative program/course proposal to the Department of Library Public Services chair before departmental curriculum committee meeting when proposal will be considered. This form will be signed and returned to proponent withing three working days.
