Undergraduate Forms
Form:	Purpose:
Associate of Interdisciplinary Studies (AIS) Degree Program Form	Students pursuing the AIS curriculum.
Certificate as Minor Request	Allows students in majors requiring fewer than 48 credit hours to declare a certificate in place of a second major or minor.
Degree Program Change Form	Changes to second degree programs.
Exception Form (For Undergraduate Students only) Additional Fields for Exception Form	Substituting a WKU or transfer course for another WKU course in the major or minor requirement. Waiving a grade/elevated GPA requirement. Waiving a course or hours in a major or minor. Manual screening of World/Foreign Language Requirement.
General Education / Colonnade in Degree Works	For students that completed all Old General Education requirements prior to re-admission/catalog term change.
JUMP Exception Form	Authorization to use graduate courses toward both undergraduate and graduate degrees (JUMP students only).
Professional Education Exception for Courses Older Than 10 Years	Allowing courses over 10 years old to apply toward professional education requirements.
Request to Change or Extend Catalog Term / Year	Updating curriculum to show new major, minor, and general education/ Colonnade requirements.
Second Degree Program Form	Students pursuing a second associate or second baccalaureate degree must complete this document.
Graduate Forms
Form:	Purpose:
Catalog Year Change Request	This request is to select newer program requirements for the program to which the student is already admitted, but not an older catalog year.
Exception Form (For Graduate Students only)	For course substitutions and transfer course articulation only; exceptions to policy should be requested on an Appeal of Graduate Policy.
